Re-design our PDF / Paper Information Pack

We are a firm which provides a compensation claims service. We have recently rebranded.

Our application forms were designed on word. We are now looking to make the forms look more professional and fun. We therefore are looking for the form to be designed professionally, and for it to be delivered back in readable pdf format.

The requirements are as follows

1) This form will be printed, so no color to the edge (our printers don't support this)
2) The address must stay in precisely the same place
3) The content must remain on the same page as it does now (page numbers must remain identical).
4) The RGB colours our business use are Blue #2f3996 Green #669348. We would be looking for CYMK equivalent to be deployed
