QUICK and EASY Membership Comparison Chart

Hello, my name is Ryan and I'm looking for someone who can QUICKLY put together a membership comparison chart, somewhat like the picture that I have attached. I only have 4 levels, and a few perks per level, so it's not too much detail. I would need this constructed in a .jpg or .png format so that I can just plug it right into my website page.
