Packaging design

I need some packaging design work done.

My existing packaging is horrible. Full color one side, written text the other side. See attached.

I currently sell to the homebrew beer market with three sizes.
I also currently sell to the homebrew wine market with two sizes.
I will soon be offering product to the jar canning market with three new sizes.

So I will need:
1. Three different designs for the three different markets
1A. with only slight changes for the different sizes in each market.
2. Any normal requirements that big retailers require in packaging that I may not know about, such as: ISBN numbers, "Made in China", etc.
3. I currently use a single fold high gloss paper insert that cups the relatively flat sheet like product inside a ziplock bag. I want to keep it that way for all markets because it's cheap. Ranging in size from 9.375" x 4" to 9.375" x 7" then folded in half.
3A. But I will also want the canning jar to have a heavy card stock envelope with retail hanger hole that holds an 7.5" x 10.25" flat product. So that it will be rigid and easier to sell in craft stores. (Much like you see in avery label packaging.)
4. My supplier does the printing and packages everything for me so I would need this in a format I can send them.

What are your thoughts, capabilities, cost estimates, etc.
This is a low profit business at this time with limited potential, so I am shopping around for the best price of a company that can handle all requirements. See the attached.
