Description

Experience Level: Intermediate
Design details: The document is 3 pages with 30-40 items per page, it should work like a "real" pdf where check boxes, fields etc can be filled in using your keyboard, and the "tab" button should jump to the next field on the form etc, should also work in mobile

I need the real deal! See screenshot of all the fields needed (attached zip file)
Ideas for the visual style: follow design already created
