I need a graphic for use on my website homepage / mailers and brochure

I need a graphic designed utilising the content and outline format i will attach. I will sit the image / graphic into the homepage of my wordpress website and i want to use it in mailers and within my e-brochure. It needs to look clean, crisp and professional.
