I need a credit-card sized loyalty card designed (front and back graphics)

To be made using my choice of colours, sample graphics will be provided to give you a feel for the current design. Willing to be surprised so you have some freedom. Front of the card is to have my company logo and the words 'Loyalty Card' in a clean, professional font. The back should have a blank section for signing your name and my company contact details and web address on the bottom. The person I choose will have all the information required e-mailed to them.
