Design Powerpoint templates from attached mock-up in Word

Please can you give me an indication of the cost for doing the following:
Attached is a 3-page Word template. Ultimately, this type of document will be populated with how-to type content and appear on a website a Factsheet of 3/4 pages in length.
What I am looking to do is to have it designed in Powerpoint (if this is the best way, I'm open to suggestions) with the objective of making it look a bit like a spiral-bound notebook.
I was originally thinking the spiral part would be at the top of the pages and have mocked it up this way. However, I am open to the spiral element being on the left-hand side if this looks better.
In essence, there will be four A-4 pages consisting of two columns on each page. All the text should be editable but ideally not moveable. Furthermore, the text in the left main columns should flow into the right-hand column. The text in the right-hand colum should flow into the next page. I don't know if this is possible, though.
There should be four pages in total:
1. Opening page with the logo on and Researcher's Notes in a large font together with the title of the Factsheet. On my word document, this is labelled 1/3. On the Poiwerpont document it should be labelled 1/4 but editable.
2. Two centre pages, primarily with space for content. The pages will be identical apart from the page numbers. On my Word document, these are labelled 2/3. On the Powerpoint document, they should be labelled 2/4 and 3/4. I would like to have the option to remove one of these pages if the document ends up only being three pages long.
3. An end page with the logo on and disclaimers etc. On my Word document, it is labelled 3/3. On the Powerpoint document, it should be labelled 4/4.What I would also like on the last page at the bottom of the right-hand column is something which looks like a post it labelled:

It should be editable.
Logo - I am open to other positions for the logo. Because the logo has an upward-pointing arrow, I have been wary of putting it top right as this may encourage the reader to look off the page.
Colour - I see the text as being standard black but am open to the rest. The "PostIt" should be a pale yellow.

The explanation may be long but once you see the Word document, you'll know what I'm on about! The only item not on the Word document is the Post It on page 4 which I've just described.
Your thoughts on how this may look are more important than doing the job quickly.
