Design pop-up greeting card

Design pop-up 3d greeting card using main graphic that I provide.
Here are specs from the printer:
1. We require high resolution PDF files (CMYK PROFILE) with bleed at the edges and trim marks required.
2. We need a separate die cut outline for the pop-up cut out including all the scoring lines, and a separate artwork for the base card.
Attached is an sample of the black Santa main graphic.
