Create small graphic to be part of website pop-up form

Design details: Need a graphic artist to develop a booklet image with a custom cover that will be used as part of a pop-up form for my business website. The pop-up will appear and the user will be encouraged to download a free copy of my free booklet. I have attached a mock-up from a competitor and would like the one designed for me to include my company logo and the following copy:

Top Headline Copy: Assess People (insert solid circle) Predict Performance

Photo To Use On Cover (attached)

Body Copy: How Assessments Help You Hire, Develop, and Retain the BEST Employees

Bottom Copy: Company Logo (attached)

**There is no requirement to replicate the font type/size of the attached sample, but I want the design to be visually attractive and attention getting.
Ideas for the visual style: Color scheme should be compatible with logo colors but also use an alternate color to add contrast. Designer has discretion to select font type/size, but it should be bold and easy to read.

Extra notes: I don't have exact dimensions the graphic needs to be, but it needs to be designed to be displayed on the web, high definition, and at least 3-4 inches in size.
