Create HTML emailers

We have two pages (including the sample I have attached) to create in html format that we can send to potential clients in the form of a responding email. Each page also has two separate email response sections that will also need some type of "responder" page where we can ask for information, and the potential client will fill out the information, and have the capability of attaching photos, text, etc. You will see those areas on the sample I've included (noted as xxxx for confidentiality, and the name of our business is xxx for confidentiality on the mockup attached..

I also have attached an emailer that we will be sending out for ongoing email "campaigns" -- so I would like a separate "quote" for this particular emailer, with the idea that if the work quality is good and the price is right, we will continue to have you create new emailers, similar in content and complexity to the ONE I have attached (the turq with leaves). We will probably be creating two to three a month, to begin with.
To summarize: I need a quote for TWO pages (same page but changing out a few bits of the info with a different name) and two responder email link pages to coordinate with these pages.

AND ALSO, a quote for the ONE emailer I have attached (turq and leaves) but with the understanding that same price or ballpark price will be charged us for future emailers, same complexity, similiar look.
