X3 months payslips for an ad commercial /short video

I'm looking for x3 payslips (November /December /January ) date is 21st and must include the persons and address as well as the companies name/address. (ask for details)

Total must come to 4,500 and has to look professional. (you must take into consideration that taxes restart every April and the gross of 75k is 6245).

Whatever deductions you have to make to get 4,500 a month is fine.

Example copies are welconed.
