Worksheet for training sessions required

Design details: the design is for a worksheet for attendees of our training sessions to use and write on. the worksheets will be printed on thick paper/slim card, and ideally be the size of an a4 piece of paper once unfolded. i am happy to let you create a fun and funky worksheet, however bear in mind this is meant to be educational .
Target audience: graduates aged 23+
Number of pages needed: 4 maybe
Content on the front/back side and body: company logo, content to be provided
Ideas for the visual style: fun, funky, edgy, yet professional
Preferred colors: gold, white, royal/navy (as attached)
Extra notes: hi, i need a worksheet provided for use in a training session for attendees.

the worksheet needs to have a front cover, and also a perforated edge on maybe a folded part of the worksheet to tear off which will serve as a feedback form. ideally there would be only one page for the notes section and cover, and the other would be the tear off feedback form. i will work with you to create the copy for the worksheet, however i need the layout nailed asap. it needs to be compact and funky, yet fun.

as you can see mine is pretty bad - i need this worksheet art work completed by 12pm this sunday so tight deadline! i will supply the proper logo and copy once the layout is finalised.
