Simple Layout Job with Typesetting

I need a "TV Card" set up using Adobe InDesign. This is a two-sided card.

Fonts MUST be MYRAID family, found here: https://www.dropbox.com/s/u07clblxqu3kvs8/Myraid%20Fonts.zip?dl=0

Please see the file titled "SAMPLE" for an idea on how this should look.

Please see the file titled "USE THESE CHANNELS" for the list of the channels. You will need to typeset these. Setting up lie the SAMPLE ... a black box with the number and then the channel name. DO NOT place a hyphen between the box and channel name.

Top of page 1 should read "Television Channel Line-Up" ... then list as many channels will fit, in two columns like SAMPLE.

Top of page 2 should read "Television Channel Line-Up (cont'd)" and then the rest of the channels should be listed, in two columns, leaving the bottom blank (do not remove the black box on the InDesign file).
