Report generator based on dropdown items (with scoring)

Hey there. I am after someone to create an Excel-based or suitable alternative report generator.

This consists of around 20 drop-down fields (2 sections), each with pre-defined answers (around 3-5 answers per drop-down field).

Each answer will need to have a score attached based on the selection (in percentages). Which in turn will produce a 2 sub-scores for each section, and 1 overall score based on both sections.


Once completed, we will need to have the ability to produce this into a PDF completed with our company branding.
