Interactive PDF Documents

Design details: DESCRIPTION
I am constantly printing and scanning documents whenever a new project starts and would like to finally move over to a complete digital process. I have a set of Interactive PDF Documents which needs to be designed and created. I have a very good idea of what needs to be designed and will provide all the content for each document.

The final product will be a fully interactive .PDF document which will be built using a combination of Adobe InDesign, Illustrator and Acrobat.
A major part of the interactive forms is the JavaScript back-end. Some of the forms will require a simple PHP script to store values into a MySQL database of which the database modelling will be created by you.
After successful submission of the PDF forms a HTML email will be mailed to the user. Either a ready made HTML script will be used or an event will trigger PHPLIST to send a test mail to the user. I would prefer the latter to be used.

This project has been merged with another in-house project, however a lot of the content and code will be reused. Here is a breakdown of the different documents which needs to be created:

Medialabs

* NDA Document - Content will be provided. Interactive fields will include text boxes and signature fields.
* Project Role & Contractual Agreement - All Content will be provided. Interactive fields such as Text fields will be place in numerous areas and separate pages. At the end there will be signature fields.
* Termination Agreement - Very similar to the two documents above. Again, content will be provided and the document will consist of interactive fields such as Text Fields and Signature Fields.
* Project and Role Change Form - This Form requires hardly any text, but alot of space to write new adjustments for a project. At the end there must be signature fields.
* Interactive Corporate Letterhead with Continuation Page - No content required and the letterhead has already been designed. All that needs to be seen is a full paged size input box.
* Interactive Invoice Template - This requires the most amount of attention as certain columns will need to add up or multiplied against one another automatically. Form elements such as a combo box may be used.

Client

* NDA Document - One version must be in Thai and another in English. Interactive Fields include signature boxes and input fields.
* Contractual Agreement - Content in English and Thai will be provided. Interactive Fields include signature boxes and input fields.
* Termination Agreement - Very similar to the two documents above. Again, content will be provided and the document will consist of interactive fields such as Text Fields and Signature Fields.
* Interactive Corporate Letterhead with Continuation Page - No content required and the letterhead has already been designed. All that needs to be seen is a full paged size input box.
* Employee Application Form - There are many free examples online which can be used, but customized to the client's design specification and make interactive.

In total there are 11 documents to create, but keep in mind the code will be reused, content is supplied so it needs to be copied and paste.

WORK FORCE
I am looking for someone who has experience with Acrobat JavaScript and knows how to design forms using tools such as Adobe InDesign and Illustrator. The database model is simple and will be designed with the Project Manager.

COMMUNICATION CHANNELS
All correspondence and sharing of files will be done within a Project Management System called Red Booth. It is preferred to use Line app for instant messaging rather than the outdated Skype application.
