I need to convert an adobe livecycle form to an editable PDF

I have a fillable PDF Form at leanbrandingcanvas.com. The form is used by startups to create a brand strategy. It is a fillable PDF and has scripting to ensure the instructions come back when the text from the field is deleted.

I have 3 small changes to the text, but the main thing is to have the form look and act as it currently does (when done properly).
