I need the progress reports for my school put into a specific design

Design details: It's already done, I just need the new reports put into the same design style.

Extra notes: The format has already been designed by a branding company. I will attach an example of the finished project, and all I need for you to do is to put the new versions of the progress reports (there are 7 in total, each two pages) into the specific design. The finished project needs to be in pdf format, and I should be able to enter a text comment in the 'Teacher's comments' section. I attach an example of the finished product, and then two of the documents that need to be put into that format.
