I need some templates set up in Microsoft Word

Information for the business: Need two templates for Avery labels
Design details: I designed a new logo for a client, and they asked for templates featuring the new logo. One is for Avery mailing labels, the other is for Avery name tags. I need to have the design elements placed in precisely the right location, and for the text to be set in the correct location and fonts/sizes. I can provide the logo and all files in Adobe Illustrator, PNG, or whatever format will work best. I can provide mock-ups of how everything should be placed and sized as Illustrator, PDF, or PNG files. I can provide the fonts and the Avery templates. I just need precise, functioning templates back to provide to my client.
