I need a professional press pass design

Information for the business: I own my own PR firm and we need a badge for people to wear covering media events
Industry: Entertainment
Design details: I am looking for a badge design that I could submit somewhere to get printed on a hard card. If you have someone that could print it -- you get preference on this.

I need the press pass to have the following information (which I will send):
1. My head shot
2. My company logo
3. A few details (such as my website, my contact information, phone number)
4. Big letters saying Press Pass
5. My signature (which I can send you digitally)
6. A few other things (see examples below)

Extra notes: If you have someone that could print it or know of where I can get it printed on a card (similar to that of a University's ID card) then please let me know. If someone could help with the printing that would be amazing. I will pay extra for the printing.
