    In what format / software format you need template made?
    I would suggest using InDesign (that's DTP industry standard).

    there is no way a professional can offer a proposal without any indication how large a job this is. Plus the industry standard for templates of this nature is Indesign. Do you have and know how to used Indesign?

    Photoshop will ruin your book and things like Word or Illustrator do not have the design capabilities you are looking for
