I need a design of a gift card and gift card holder

Information for the business: We sell fine wines
Industry: Food and Beverage
Design details: We need a gift card and gift card holder to be designed.

The gift card will be printed on a standard plastic card (credit card format) and will be placed in a gift card holder which will be a A5 folded A6 (like a birthday card)

The gift card will have on the front: Our logo and a random number embossed on the bottom right corner
On the back there will be the following mention: Present your gift card for payment and the amount of your purchase will be deducted from the balance on the card. This card will expire 12 months from date of activation. At expiry, the balance will be lost. This card cannot be exchanged for cash, returned or refunded. This card cannot be replaced if lost stolen or damaged.


The gift card older will have
on the front page: Our logo, and the word "gift card"
on the second page and nicely designed section where the person purchasing the card can right details. It needs the following sections: Value, for, from, and a message space
on the third page the card will be glued on and our monogram will be uncovered when the card is removed
on the last page: our company details

All guidlines and fonts will be provided
Content on the front/back side and body: Title, logo
Ideas for the visual style: Very smart and minimalist
Preferred colors: colors spectrum will be provided
