I am looking for someone who could design an online "voucher" template in word

Design details: I need someone who can design an online voucher that will we advertise on Amazon. These vouchers will be selling consulting support, technical support or training, and purchasers will have options to purchase hourly "packs" in one, three, five, ten, twenty, fifty hour options. The vouchers we show on Amazon will be "dummy" vouchers. The actual vouchers we issue will have spaces for "unique serial number", "referrer", and "expires on" fields. The design needs to reflect our corporate image and will be published to PDF format before being issued to users.
