Graphic Design Post Card

I need a 5"x7" post card designed by Friday (6-7-18) or sooner. This card will be used when I go to events to autograph for kids.

I attached a .jpg example of a card I like. I want the same I attached a .jpg photo of myself that I want to use on the card.

I want my name to look like the font that is 2017 NATIONALS but instead I want it to say: CHRIS POULOS and leave the rest blank so I have a place to autograph the cards but where the website is I want my website: www.chrispoulos.com

Where it says Lucan BMX I want it to say: World Champion

Where the bottom corner is I want my social media. I am not sure of the best way to make that look? But I have YouTube, Instagram, Twitter, and Facebook. All my social media is chrispoulosbmx

The colors don't need to be exactly the same and the wording doesn't need to be in the same places. If you think it looks better someplace else. Also, if you have any better design ideas I would be open to something else if it appeals to me.
