Fillable PDF form design

I require a PDF fillable form to be re-created in Adobe DC or another professional PDF form maker (not Adobe illustrate or graphic software).

The form is 5 pages, all text and colours are set. However the person that created this has try to cut corners and initially converted a Word document to a PDF, and then in a form editor they have added text boxes and radio buttons as overlay. As a result the form does not look professional.
Current issues with form:
• Editable Text boxes – these are separate overlay text boxes to the background text box. This doesn’t look good or professional. It should all be embedded correctly
• Circle Radio buttons – these are overlayed on the background radio buttons. This doesn’t look good or professional
• Circle Radio buttons – there is a blue square shade over the circle radio buttons. This looks poor as it doesn’t match the circle radio button.
• Check boxes – these should be tick boxes. Instead the check boxes check a small square bullet point
This is a relatively easy job for someone experienced in making forms. Please don’t use this project as a test, and please do not put yourself forward if you don’t have experience – graphic design does not count and this current distorted form was created incorrect by someone who does graphics.
