Edit Time Card in Adobe - read before bidding

I need a time card edited in Adobe. Time card has electronic signatures, drop down dates, etc. I have attached two pics to show what I need to have done. If you accept project I will include source file of time card and and Excel sheet showing calculations. If easier , you can you excel sheet and create time card in adobe from it. Finished project should be able to still be signed in Adobe Reader DC, and still have all the functions of the original source time card (which I will provide).
