DVD Menu Design/Animation

I have a project requiring a high quality DVD menu, to include a short animation introduction video, then a simple menu with links to 8 videos. (these videos will be provided as rendered MPEG files) This menu most look very professional, smooth, and include a few touches to impress the viewer...animated buttons, backgrounds etc.

This will be an ongoing requirement for other video projects in the future. I would like a \"per DVD price\", and some examples of work previously completed, and a price expectation for the examples. Please let me know what software you will be using for the authoring.
