Design of Loyalty program (flyer, cards, registration form)

We are looking for somebody to design the style for our loyalty program stationery.
It needs to be refined and creative, matching with our company design/logo. All the different items should be coherent in design with each other; we're looking for the same style.
The project is comprised of the following:

- Loyalty program logo
- Registration Sign up form (A4) with terms and conditions
- Card design (PVC card, 2 categories, front and back)
- Loyalty program information flyer (trifold A4)
- Rewards redemption list (A4 design)
