Damp, Timber & Flood Survey and Quotation Form

Hi , I require a professional, double-sided A4 form (one sheet, both sides) for use on-site by DrySure Restoration Ltd surveyors. The purpose is to efficiently scope and quote for damp, timber, and flood-related works in residential and commercial properties.
Key requirements:

- Format:
- A4, double-sided (both sides of one sheet)
- Incorporate our existing logo and company branding/design elements

- Content:
- Use the revised text provided (attached Word document), which includes all required form fields, checklists, and sections
- All fields must be easily writable by hand on a printed copy

- Plan Section:
- Approximately half of one side should be reserved for a “Plan (not to scale)” area
- This area should feature a faint graph paper/grid background to allow for hand-drawn floor plans and property sketches
- The “Key” for plan symbols should be overlaid on the plan area (e.g., as a semi-transparent box or condensed section in a corner), to save space and keep the form within two A4 sides

- Design Considerations:
- The form must remain clear, uncluttered, and easy to use on-site
- All text and fields should be legible when printed in black and white
- Graph/grid lines for the plan area should be light enough not to interfere with handwriting or plan details

- Deliverables:
- Print-ready PDF of the final form
- Editable version (Word, InDesign, or similar) for future updates

Notes:
If space becomes limited, prioritise keeping all form fields from the provided text. The plan area and its key may be condensed as needed, but should remain functional for field use.
