I need an urgent Excel form to be created for a medical form. Attached are the files. The chart named 'Ellis Hospital Chart to Excel FORM' is the one which needs to be cleaned up and created as a neatly done Excel form.
Also, the left section of the form might not be clear, so thought will attach the left section along as a seperate JPG as 'Left block of the chart - Better View'. Also, the older form in PDF is attached for reference as some of the fields were borrowed from it, thought might help during design.
Am hoping you can pull this off in one day, would be great. Let me know.
Number of pages needed: 1
Content on the front/back side and body: Only front.
Ideas for the visual style: Already given part of the attachment
Preferred colors: light green.
