Adobe Captivate E-Learning Project

I need assistance with putting together a basic e-learning module. We already have the content and need a nicely designed layout. I need the cptx file so we can plug in our video content but want the template to be professionally designed and have the following areas:

1) Cover Slide
2) Introduction to Course
3) First Course Section
4) Interaction (drag / drop correctly to proceed)
5) Second Course Section
6) Interaction
7) Third Course Section
8) Interaction
9) Fourth Course Section
10) Interaction
11) Fifth Course Section
12) Final Exam which will be TEN questions True/False and Multiple Choice (just create the placeholders)

The course sections are ONLY inserted video that I'll provide after the fact and won't be your responsibility.

Not sure if its possible, but I'd prefer an outline that is expandable on any page so user's can see their progress?

CRITICAL -> I want the BEST that captivate can produce here, send screenshots of similar course designs you have done in the past. You can even use a similar format/design, I have no issues with that at all. I just need a professional layout.

This SHOULD be a responsive design/layout as well

IF it's possible -> I want to "force" the user to go through each slide before they can go to the next, but have rewind/forward controls at the bottom (So making sure they watch the video, successfully complete the interaction, etc etc)

Please send designs or screenshots of similar course layouts you've completed and a fixed price. We will fill in all of the content, we just need a proper template designed that covers the above.
