Add form fields to word docs

I have 4 cotracts, all pretty much idenrtical, with very few fields and a signature block... I just need form fields added and saved as pdf's (that I can modify and also import to word) this should not take very long, I just dont have time to do it. I have attached an example - are you able to knock these out for me real quick? If so, how long do you think will take? I was thinking since its only a handful of fields and theyre all pretty much identical, would only be like an hour but let me know. Also, how quickly can you begin/finish?
