A cover for a small baptism booklet

I am printing a small booklet for the baptism of my daughter. I was able to format the pages within the booklet, but need a graphic designer to design the front/back cover. The booklet will be A5 so the design will be on A4. It is a super simple project and I know exactly what I want - please refer to upload. The format is exactly the same, simply colours, text and image will change. I believe this is a 1 hour job and quite straight forward. I am in a rush (2 day turn around).
