I need a new design sale receipt

Hi

I need a sale receipt. I have uploaded a document. It should be the same, only thing is it needs for me to be editable, so I can add address and other information when I need

The sale receipt template should be A4 paper size and also another should be A5 paper size, so there should be two on one A4 size paper

The sale receipt should be word, pdf and excel format.

Note

Sale Made with, should be changed to Paid by
Check, No____________ should be changed to Cheque, No________________
The boxes [ ] should be changed to a square box
As the sale receipt, it should be in a box
SALE RECEIPT should be in capital letters, underneath it should be editable for me to put address and other information on either side
After the last Paid by box, there should be space for me to add or editable information.
Please make sure other things are as it is on the document
