Workout log books for clients (personal Training)

Im a personal Trainer and want personalised log books for my clients to help them track there workouts.
the project itself is quite simple. I have a logo already to go on the front and will attach a sample of the kind of thing i want.
Ive already done most of the content but my lack of excel and word skills were making it difficult for me.
Someone who knows what there doing can probably do it in a few hours.
I have a company who will print so i will need to send you there margin spec etc. The book is to be A5 in size
