Silk Screen

I'm looking for someone to make me up a silk screen for printing some text onto fabric, I have attached a file showing the writing that would be needed, the text needs to sit inside an 11" square area, I will send the correct sized text through to the winning bidder. I would prefer a fixed price rather than an hourly rate, and please include postage costs in your bid.
