Layout a fairly simple 16-page ebook

The job is laying out a fairly simple workbook. It's about 3500 words, spread over 15 pages plus a cover. I will provide the doc in Word or Open Office format.

Most pages include some text and boxes for the reader to write in. There are a few simple tables. I have put all boxes and tables in place, but they need formatting, and the space etc around them sorting out.

There are a couple of pictures included.

I need a simple cover to be created, using a picture, logo and some text.

I'm looking for a clean, simple design. My ideal style is modern/classic.

Each page needs to have a brand colour and my logo on.

I will supply brand colours to be used.

Timetable:
Document to you: probably this evening or by 9am tomorrow morning
Back from you as word document and pdf: by 10am UK time Friday morning.
My corrections back to you by 2pm UK time Friday.
Your corrections back to me by 9am UK time Saturday morning. I'd ideally like you to be available in case there are any late changes at some point over the weekend.

I'd be available by phone tomorrow for questions.

I'd like it as both a word document and a pdf.

I'd ideally like to see some examples of layout work you've done, if you can direct me to a website.
