I need a e-book/book cover created

Design details: I need a book cover designed. The book is a handbook for drama games/exercises. It should be slick and professional. I like themes/colours like the following:

Spine width: 4mm Bleed: 3mm Margin for front and back covers: 6mm Margin for spine: 2mm
Total width with bleed = 290mm

And a separate front and back cover in PSD format
