Create an interactive pdf

Design details: I need to create a staff benefit interactive pdf that can be sent to employees via email.
It will have 5 main headings - My health, My Wealth, My Travel, My Recognition & My Treats. Under each heading it will have between 2 and 4 sub headings which will link to an explanation and maybe some embedded pdfs or hyperlinks to webpages. A set of icons for each subheading will need to be designed (15 in total) Examples will be supplied.

I am looking for a partner who can provide 1st draft by Thursday 25th August Close of play I have attached a raw pptx graphic that may help with this description and shows the colour palette, although I am looking for creativity in the final look and feel.
