A book cover and layout, sample pages

I am preparing an anthology of poems with a particular theme.
I need a cover design and someone with book experience to design and lay out the introduction, list of contents ,sample pages in clean typeface, etc,.
The best book in my area is
"A treasury of poems for almost every activity" by Estri and Kelly.,details of which can be seen on Amazon.
I do not propose to self publish, but need something professional to show to prospective agents etc.
