Personalised 20th Century Fox intro

I am looking for a personalised version of the 20th Century Movie intro. Instead the three lines would read
Mike's
80th
Birthday

I would want them to be scaled appropriately to fit the space. It would be in HD quality with the sound track from the 20th century intro. File format would be mp4 so that I can add it to an imovie.

I understand that there are editable formats available on software platforms such as blender, so hopefully this should be relatively easy to do.

I would want this for end of day on Tuesday.
