Design of Online shop

We are a start up company in online sales for optical products. I am looking for someone who can design an online shop from any standard template but I have some specific requirements on couple of features on the website to comply to regulations.

General pages such as Policy, T&Cs, About us, Contact Us

1. I should be able to list the items in 3 different categories and shoppers need to be able to search for the same item by choosing any of the 3 categories.

2. Should be linked to 2 paypal accounts (see point 4 below)

3. On check out, the shopper should be able to provide his prescription details by choosing from a drop down menu.

4. On checkout, shopper will have 3 options before paying and he need to choose only one of them
a. An option to be able to upload the prescription in any format (payment will be sent to 1st Paypal Account)
b. Or he chooses option to contact his optician (payment will be sent to 1st Paypal Account).
c. Or does not provide any of these (payment will be sent to 2nd Paypal Account).

5. Option to enter promotional code for discount

6. Be able to get pricing in GBP, Euro and AUD

I need to be completed withing 20 days.
