Design a professional looking but simple voucher in pdf format - urgent

I am a photographer. I have had a request from a client to produce a voucher for a portraiture setting so she can give it to a friend as a Christmas present. I would do it myself, but I want to avoid any feeling of it being "home made." I want it to look commercial and professional. There will only be a few lines of text, and I believe it should not take any longer than an hour, so I feel £25 is a fair amount. The format will be pdf. My client will then print it out. If there are any considerations relating to printing, please let me know.

The deadline is 1pm Thursday 20 December 2012 (London time). I would expect anyone appointed to provide me with a draft for which I can give feedback, leading to a final version. I look forward to working with you!
