Design a Logo and an Order Form

We're looking for someone to design a logo for our new product and also an order form. We'll provide most of the art work for you to play with. You need to have a good idea about fonts and typography. A proven track record of logo design would be great. If you like being different and creative and unusual, then we'd like to hear from you.

Our deadline is tight, so we'd prefer to work with someone who can get the job done by the end of the week.
