Design a leaflet mainly to use on our web site

I need an A4 leaflet mainly that we can adapt for our easiphone web site;
We need a sales/info type leaflet to promote our 24/7 "telicare" service,please see details of this service on our web

We need a more slick page with better visuals on how the service

Also need to mention,
