Customized State Map

Here is the desire:

I would like to have 4 Maps created, very similar to these: http://www.etsy.com/listing/115274653/customized-massachusetts-8-x-10-state?ref=shop_home_active

These are my requirements:

Countries:
USA - Massachusetts - heart place in Boston
Great Britain - heart placed in specific UK location
Barbados - heart place in Ocean City
Bermuda - heart placement tbd
Germany - heart placement tbd

I would like the country to actually be in one of two colors (orange and then a specific blue similar to the Tiffany Jewelry blue), the background should be just plain white.

If you tell me where to go print it (UK only, I am in London), I am more than happy to just get the designs via email/pdf and then print them myself if that makes things easier.

Deadline would be a week from today if possible but if I am the one printing them, I have some more flexibility. I would just need some guidance where to go.
