Creation of a 2-page website

I own a domain name and have designed and built my own temp webpage through one.com. My website is and will be a business directory but with a difference

My current website is just a temporary one as i designed it 2yrs ago. But now i want to start building the company up. Just for now i want a simple 2 page website designed (it will get bigger) alowing business to fill in a form of their company & contact details and select an option of what package they would be interested in and then when entered all information be stored for my telesales to contact at a later date.

The second page will show the packages that are available to them.

I will require this to be done asap and they you will need to be able to host it and do all the necessary communication to bring my domain name across from one.com (as i have no idea)
