Create true 3d image

we require a rough idea of an image making into a proper 3d image from the perspective required eg looking upwards from bottom left this is important that you ar looking up from here.
this is required asap
please do not bid if you cannot produce a nice smooth looking 3d object with smooth lighting effects. we have adobe master collection 5.5 so illustrator output would be ideal.
