Create PDF survey forms

I have hand drawn a survey document which consists of approximately 10 pages which I require somebody to turn into a PDF document for me.

The document will then be used by operatives in the field when carrying out surveys, entering info into the text boxes on the document, selecting items from drop down box, and clicking on tick boxes.

I can send the hand drawn proposals to anybody interested in providing a quote to create the documents.
