Create an InfoPath form to collect a selection of data via email

I am looking for an InfoPath designer who can put together a form to be completed by email. We have details of the fields we need, what the data needs to look like in the fields and we know that we will mail merge the form and ask our suppliers to fill it in by email.

I don't imagine it would take someone with experience very long at all as there are only 75 fields (mostly requiring yes/no answers). We would also require our logo and short introduction text at the top of the form.

The biggest technical challenge will be that we need the option for the user to repeatedly fill out a section of the form for as many objects as is necessary.
