Create a facebook style timeline with a poll for each entry.

I have implemented a rough system that achieves what I want it too, however; I would like the following modifications.
1) I need the current implementation of this software to work correctly;

2) I want a simple script that creates a new poll like the one from "mypollhost" but which looks neat and most importantly uses the local MySQL database rather than some other host.
If you have a better plan just send me a brief with your bid.
I would like this done very soon.
